EXPRESS CONSENT AND AUTHORISATION to Via Moda, S.L. (hereinafter Via Moda) to: • Obtain my personal data in order to: inform me of special offers, include me in a variety of loyalty programs, send me news and information related to Via Moda and give me access to exclusive in-store offers, • Process the data obtained for the purposes mentioned above. In compliance with art. 13 of Law 15/2003, I DECLARE TO HAVE BEEN INFORMED: 1) That the personal data provided will be included in the client file (PM0201) owned by Via Moda, duly registered with the Data Protection Agency of Andorra, and will be processed for the established purposes set out in this file and protected by the mechanisms specified therein. 2) That I will be able to exercise my rights of access, objection, rectification and erasure (to be forgotten) with respect to Via Moda (being the party responsible for the processing of personal data). 3) That the party which will receive my data and be responsible for its processing will be Via Moda, S.L., domiciled at Carrer de l’Aigüeta, 2-4, Andorra la Vella. 4) That the data will be kept indefinitely. 5) That the consent given here can only be revoked if the signee addresses a request in writing to this effect to the party responsible for the data processing wherein this revocation is clearly stated.
